The differences of the postoperative process in elderly after laparoscopic and laparotomic chlecystectomy

Objective: The aim of this research was to investigate how the laparoscopic and laparotomic cholecystectomy affects the postoperative period in the elderly.
Methods: The target group consisted of 59 patients over 65 years after cholecystectomy who were hospitalized from January 1st 2010 till December 31th 2010 in Vitkovice Hospital a.s.
Results: We found that laparotomic cholecystectomy is less convenient for the elderly. The open cholecystectomy in elderly leads to a gradual mobilization and self-sufficiency, to increased incidence of postoperative nausea, vomiting, a higher incidence of postoperative complications, slow recovery of intestinal motility and defecation, gradual realimentation, longer period of hospital stay and greater intensity of postoperative pain. In elderly, after the laparotomic cholecystectomy made, we found higher rates of introduction of drains and permanent urinary catheters.
Conclusion: Postoperative nursing care should focus on a thorough knowledge of elderly and their motivation. Simultaneously, it is also necessary to ensure the quality of nursing care. In patients after laparotomic cholecystectomy, the nurse should put emphasis on mobility of patients, which would lead to earlier return of self-sufficiency. Simultaneously even early mobility along with realimentation should lead to faster development of intestinal motility and to first postoperative defecation. The nurses hould prevent complications.
